Social Workers/Discharge Planning

In the event that personal non-medical difficulties arise while you are in the hospital, please feel free to speak with a staff social worker. These professionals can help your out-of-town family members make temporary living arrangements in Louisville. They also can answer questions about community resources and outside services.

The social worker also is responsible for coordinating all agency services that you will need upon discharge, and would be happy to assist you and your family. Effective discharge planning begins upon admission. Your nurse can arrange a consultation for you.
